207 • The consequences of failing to file a report. • Special assistance in completing and filing periodic reports to households whose adult members are all either mentally or physically handicapped or are non-English speaking or otherwise lacking in reading and writing skills such that they cannot complete and file the required report; and • A telephone number (toll-free number or a number where collect calls will be accepted outside the local calling area) which the household may call to ask questions or to obtain help in completing the periodic report. SNAP households in which all adult members are elderly or disabled with no earned income are assigned a twenty-four month certification period. Under simplified reporting rules, these households are required to report any change in the household’s circumstances only at the Interim Contact (DSS Form 3808) or at the 24-month Recertification (DSS Form 3807A), unless the household’s gross income exceeds 130% of poverty or a member of the household has won substantial lottery or gambling winnings. SNAP households in which all members are elderly with no earned income (ESAP) are assigned a 36-month certification. Under simplified reporting rules, these households are required to report any change in the household’s circumstances only at Interim Contact (DSS Form 3808) or 36-month Recertification (DSS Form 3809), unless the household’s gross income exceeds 130% of poverty or a member of the household has won substantial lottery or gambling winnings. Changes must be reported within the first 10 days of the month after occurrence. The DSS Form 3808 is used as the change reporting document for annual reporters. The DSS Forms 3808 and 3809 are mailed to households and must be returned to the Agency by the 15th of the month the respective form is due. Incomplete forms which contain the household’s signature, name and address or forms filed by the authorized representative must be accepted. See Section 15.6 Action on Interim Reporting Households for the Interim Contact Process and Section 19.13 Determining Continued Eligibility for ESAP Households for the Recertification Process for ESAP SNAP households, which are assigned a 12-month certification period, are subject to simplified reporting requirements at each recertification. These households are required to report any change in the household’s circumstances only at the annual recertification, the six-month recertification or the tri-annual recertification, unless the household’s gross income exceeds 130% of poverty, an ABAWD’s work hours fall below 20 hours per week or a member of the household wins substantial lottery or gambling winnings ($4500 or more). This change must be reported by the 10th day following the month the change occurred. Although the six month or tri-annual recertification does not require an interview, either face-to-face or telephone, these households do have the right to a face-to-face or telephone interview if one is requested by the household or its authorized representative. See Chapter 17 Recertification of SNAP Household for the Recertification Process for regular SNAP.
