923 Chapter 5:Temporary Assistance for Domestic Violence Survivors (TA-DVS) Program • Section 9: Co-located domestic violence advocates (07/2026) certification period. Verbal Authorization ODHS may accept verbal authorization on the MSC 3010 when a hardcopy signature cannot be obtained to provide consent from the individual to share limited DV information with community agencies only. Verbal authorization is only to be used to share the below information when related to DV. The only information to be shared when a verbal authorization is used is: 1. The individual’s name 2. The individual’s safe contact information 3. Confirmation that the person wants the community agency to contact them. Recipients can only be community agencies. Verbal authorization cannot be used for any other DV information sharing purposes. The risk of phone conversation being overheard by someone else b. The individual will not have a written copy of the MSC 3010 4. The MSC 3010 is completed when ODHS staff: a. Reads the text to the individual and asks the individual what they would like to have written. b. Reads back to the individual each part of the completed MSC 3010 to confirm it reads as the
