809 Chapter 4:Working with families • Section 6: Child support programs (07/2026) existing support order against the mother or father of a minor parent receiving TANF, OCSP will only pursue support if the minor parent is living with a caretaker and is coded as a child on the grant. However, in all cases, ODHS reports the absent parent of the minor parent’s child to OCSP. Paternity cases: Except when paternity has already been legally established for all children in the benefit group or when a participant has been granted “good cause,” ODHS may ask the participant if they would like to voluntarily complete a Parentage Declaration form (MSC 0112FLS) for each child included in the benefit group. If the participant would like to complete the Parentage Declaration form, ODHS asks if they would like help completing it. ODHS cannot require a participant to complete this form. This form is available via a link on the ODHS Forms Server or the DOJ website DECLARATION IN SUPPORT OF ESTABLISHING PARENTAGE A guide for assisting participants in completing this form can be found in Staff Tools-TANF-Child Support. OCSP may require this form once they process the TANF referral or update. Voluntary Acknowledgment of Paternity: ODHS uses the Voluntary Acknowledgment of Paternity Affidavit (OHD 45-21) instead of the Parentage Declaration (MSC 0112FLS) in all cases where: • Paternity has not been legally established, and • The child was born in Oregon or Washington, and • The mother was not married when the child was born, and • The child was not born within 300 days after the mother’s marriage terminated, and • Both the mother and alleged father are present, have had their rights and responsibilities presented to them and willingly sign the Voluntary Acknowledgment of Paternity Affidavit to establish the alleged father is indeed the legal father. According to federal law, parents must hear the “Statements of Rights and Responsibilities” printed on the back of the Voluntary Acknowledgment of Paternity Affidavit. Staff may read this statement out loud to the parents, or parents may watch the DVD provided by the state and available on the OCSP website at http://www. oregonchildsupport.gov/paternity. This form must be notarized. ODHS sends the completed Voluntary Acknowledgment of Paternity form by regular mail to OCSP at Oregon Child Support Program, PO Box 14680, Salem OR 97309. (This form cannot be emailed.) The Voluntary Acknowledgement of Paternity form and the Birth Record order form (required) is available in PDF via the OHA website. If participants do not have the ability to print they can request blank copies of the Voluntary Acknowledgment of Paternity form and Birth Record order form by emailing Kristen.farrell@DHSoha.state.or.us. When requesting this form, include the number of forms needed, whether the form is needed in English or Spanish and the address to which the forms should be sent. Paternity cases with more than one possible father: If the participant names more than one man as being a possible father for the child, the participant may be asked by ODHS if they would like to voluntarily complete the Parentage Declaration, for the man who the participant thinks is the most likely father. The participant will list the other possible father(s) in the appropriate section on the form. If OCSP or a court finds a man named on the Declaration is not the father, OCSP will ask the participant to complete a new Declaration for the next most likely father.
