711 Chapter 3:Processes and procedures • Section 5: Program integrity (07/2026) information and reports, subject to certain limitations as outlined in 461-105-0060(6), 414-175-0097 and in the Privacy Rights Policy AS-100-02. If ODHS|OHA denies access because of a good-faith belief that a disclosure could cause harm to the individual or another person, the decision to deny must be made by a licensed health care professional or other designated staff; ODHS|OHA must make a review of this denial available to the individual. If the individual wishes to have this denial reviewed, the review must be done by a licensed health care professional who was not involved in the original decision. When an individual, or person or entity to which they have given written authorization, requests access to their own case record, use the MSC 2093, Access to Records Request Form. Except for HIV information, case record information may be requested by the individual and released to the individual by telephone if the individual can verify their identity. If the individual is requesting physical copies of case record information, an ODHS|OHA staff member must be present while the individual or authorized representative has access to original copies of the case record. An individual does not need to complete a release of information form to access their own records. In response to individual requests for verification of benefits received, the Supplemental Nutrition Assistance Program (SNAP) Benefit Verification Letter (ODHS 0839) or the Notice of Income and Benefit Calculation (ODHS 7294) ) should be used for most purposes. If these options do not meet the need, some ODHS|OHA screen prints from ONE are allowable as an alternative. However, ODHS|OHA does not have the authority to and should never disclose screens from other agencies to individuals. This includes such screens as the Social Security Administration, Employment Department, and SAVE (Systemic Non-citizen Verification for Entitlements program). Note: SAVE results must be uploaded to the ONE Electronic Case File (ECF) for audit purposes. Uploading results ensures that anyone auditing can verify the results at the time of the certification that made the individual(s) eligible for program benefits. All ODHS|OHA screen prints that are allowable must be redacted as earlier outlined. Examples of what’s allowed: • Printing a department form (For example, 415F) and redacting all SSNs that do not belong to the individual or their minor child. • Printing a copy of the individual’s or minor child’s Social Security card(s) they previously provided to us from their file. Examples of what’s not allowed: • ODHS|OHA creating a letter or other document that lists the requested SSN. • Copying a department form (For example, 415F) that includes their ex-husband’s SSN and providing it to the individual unredacted. • Providing an ODHS generated system print that include SSNs. Disclosure of individual information to family members ODHS|OHA is generally required to make the minimum necessary information in the case record available to a family member if the family member is a member of the filing group or is authorized by a member of the filing
