1126 Chapter 7:Worker guides • Section 6: MSP worker guides (07/2026) MSP worker guides Introduction While the chapters within the Oregon Programs Eligibility Notebook are intended to cover all areas of eligibility, there may be miscellaneous pieces of information that do not fit into a single topic or that supplement other topics. The Medicare Savings Programs (MSP) worker guides are available as a tool to accompany the rest of Oregon Administrative Rule and the Oregon Programs Eligibility Notebook for these areas. This Section of OPEN is a work in progress. If you cannot find what you need here, please check Quick Reference Guides (QRG). Other resources Non-MAGI Tasks: Contains information on many Non-MAGI tasks and task types for Eligibility Workers, including Benefit Decrease Review, PMDDT Referral Response, SELG Updates, LTC Hardship Approval, etc. Medicare buy-in Buy-in refers to the process of the state paying for either the Part A and/or Part B Medicare premium for an individual. To be eligible for buy-in, the individual must first be receiving or be eligible to enroll in Part A or Part B of Medicare. Oregon sets up buy-in for individuals receiving QMB, SMB, or SMF benefits. Note: Oregon does not pay Medicare Part B premiums when an individual’s LTC service liability is more than the current Medicare Part B premium. Other resources Medicare Enrollment and Buy-In: Describes the process for facilitating Part A and/or Part B premium buy-in and Medicare enrollment. Part A buy-in Receiving Medicare Part A hospital insurance is an eligibility requirement for all Medicare Savings Programs (QMB/SMB/SMF). Individuals that have less than 40 work quarters may have to pay a significant premium for the coverage. QMB recipients are a required buy-in group for Oregon. That means we have to pay their Part A premium if they have an adjusted income of less than 100% of the FPL. Oregon does not buy-in Part A for any individual who has adjusted income over 100% of the Federal Poverty Level (FPL). Note: DW is another QMB program that pays for the Medicare Part A premium. However, DW recipients are not eligible to receive OSIPM at the same time. Because every individual eligible for DW would also be eligible for OSIPM-EPD (even if they choose not to receive it), they cannot qualify for DW. That is why we do not have any DW recipients currently in Oregon. Part B buy-in
