1061 Chapter 7:Worker guides • Section 1: Multiple program worker guides (07/2026) 3. A DHS 7470 is required to change to any other program(s). If a DHS 0415F was used, no new application is required as long as the requirements in OAR 461-115-0050 are met. More information on program changes When there is a change in the caretaker relative for TANF, a new application must be filled out, unless the caretaker is included on the current application. The following are examples of situations where it is up to the worker to decide if a new application is needed: • Reconsideration of eligibility on cash, MSP, or OSIPM cases denied or closed in error within the last 30 days; • Restoring a suspended case; • Changing the basis of deprivation of TANF; • Changes in any program, except from an adult program to ERDC or TANF if:
There is sufficient information available to make an eligibility determination for the new program based on the current application form and other documentation provided by the individual; AND
Everyone in the filing group is included in the application. Staff may use various forms to document the program change. The documentation to support ongoing eligibility or a change in program must be available in the case record. Options for documenting eligibility include: • Change report form Change Report (DHS 943); • Application for Services (DHS 415F); • TRACS or Oregon ACCESS narration; • ONE case note; • Written statement from the individual; • Employment Related Day Care (ERDC) and Supplemental Nutrition Assistance Program (SNAP) Application (DHS 7476). Returned mail All individuals must provide a valid mailing address during the application process. It is important to always make sure we have updated contact information. Do not use a ODHS/OHA office address as the individuals mailing address. General Delivery may be an option in some areas. More information can be found in Chapter 2, Section 3 od OPEN regarding various way to enter addresses in ONE. Even though we take steps to ensure a mailing address is valid, any returned mail needs to be handled carefully. Encourage individuals to sign up for electronic correspondence through the Applicant Portal. Mail will be located in their Message Center. This will ensure the individual is able to access all ONE issued notices. No action is taken by staff or ONE to close benefits for returned mail. Workers can continue to follow the guidance of the Returned Mail QRG linked below for guidance and steps of the returned mail process.
