1058 Chapter 7:Worker guides • Section 1: Multiple program worker guides (07/2026) Department any right for reimbursement. Responsibility for identifying and reporting personal injury settlements Individuals and their lawyer, if any, are responsible for reporting filing of a claim when they are involved in a vehicle accident or a personal injury situation to the PIL Unit and the individual’s Coordinated Care Organization (CCO). If the vehicle accident or personal injury occurred before applying for public assistance, individuals must report such a claim at the time of application. If the claim was settled prior to applying for benefits, the individual does not have to report the injury. Individuals must pursue all available resources, including any third-party resource that may be liable. Individuals are informed of their responsibility to pursue available resources when they sign the application. The Personal Injury Liens (PIL) Unit is responsible for identifying situations where an individual may be involved in a vehicle accident or a personal injury situation when the individual fails to report such situations. PIL is also responsible to collect information the individual is required to provide. PIL then uses that information to file a lien to protect the right to reimbursement. ODHS|OHA staff are responsible for ensuring individual cooperation in making the required reports with the required information. Assist the individual, as necessary, in completing the forms. The preferred method is to submit a report online through the secure web portal at www.reportinjury.org. After completing the online submission, a receipt will be emailed to the individual who submitted the report. This receipt can be used to show the date and time of the submission. The PIL Unit may provide assistance, when needed. Alternative reporting methods for an individual are to complete the Vehicle Related Personal Injury (MSC 451) or the Non-Vehicle Related Personal Injury (MSC 451NV) form. The forms may be sent to the Personal Injury Liens staff in one of the following ways. • Electronically to: personal.injury@odhsoha.oregon.gov • Mail to: Personal Injury Lien Unit PO Box 14512 Salem Oregon 97309 • Fax to: (503)378-2577 Note: PIL does not require a signed form. Sources of information Personal injury settlement situations may be discovered by the ODHS|OHA from the following sources: • Information from the individual. • Other sources of information, such as the individual’s attorney, local newspaper articles and citizen phone tips. • MMIS report. • Notice from the PIL Unit, when any of the following has been discovered:
