______________________________________________________________________________ DIVISION OF WELFARE AND SUPPORTIVE SERVICES Child Care Policy Manual The Client Agreement outlines the rights and responsibilities of the client/AR, the provider, and child care staff in reference to the Child Care Development Program. It provides detailed information about changes the household must report during the certification period and the repercussions for failing to report such changes. It also gives the client information regarding the penalties for making false or misleading statements or concealing/withholding facts to establish or maintain program eligibility. Clients and/or ARs must sign and date the Client Agreement prior to subsidy benefits being approved. A new form must be reviewed and signed/dated at each application. The original signed document must be kept in the eligibility case file and a copy provided to the client/representative. 123.2 Appeal Process Clients and/or their authorized representatives have the right to a hearing if they are not satisfied with an action taken by the Child Care Development Program that affects their subsidy benefits; this includes the assessment of an overpayment. A hearing request may not be granted by the CCDP/DWSS Hearing Officer when: • The request is not received timely; • The sole issue is either a state or federal law requiring an automatic grant adjustment; • Benefits are reduced or terminated as a result of a mass change without individual notice of adverse action; • No negative action has been taken. A hearing request must be received: • Within 90 days of the date of the Notice of Action; and • The request must be submitted in writing. The Conferences and Hearings Information Sheet (Form 6093-AQC) must be provided to the client and/or authorized representative at each application. 123.3 Voter Registration Application All CCDP/DWSS, and CCR&R agency staff must ensure the client has been informed that by completing the voter registration section of the application or declining to register: • Will not affect eligibility or benefit amounts; • The decision is confidential and used only for voter registration purposes; • A complaint can be filed with the Secretary of State, Capitol Complex, Carson City, Nevada 89710, if they believe someone interfered with their right to: o Register or decline to register to vote; or o Privacy in deciding whether to register or apply to register to vote. If the household answers “Yes”, provide a Voter Registration Application and explain: • Assistance, upon request, will be provided in completing the voter registration application form; • The registration application may be completed in private and mailed at their convenience; and • Information regarding the office where the form originated will remain confidential and will not be used for voter registration purposes. If the household member answers “No”, request they sign and date the form indicating their declination. 100-Program Overview [11] MTL 01/24 July 1, 2024
