Verify the individual’s age at the initial eligibility determination and anytime conflicting evidence indicates an incorrect age.

Acceptable verification is documentation from a third party that substantiates the individual’s age.

Note: A completed BFA Form 770, Reimbursement Agreement and Acknowledgment , (including all required signatures, dates, and notarized) must be received prior to financial assistance being approved.
