155.03 Termination at Redetermination (AAM)

Financial assistance will be terminated when any of the following apply:

· The individual is participating in the mail-in redetermination process and fails to return a completed BFA Form 800R, Application for Continued Eligibility for Financial, Medical, Child Care and SNAP benefits ;

· Complete and return BFA Form 771A, Nursing Facility Care Renewal ;

· The individual is participating in the online redetermination process and fails to complete the online Redetermination Application without good cause;

· The individual no longer meets eligibility criteria;

· The individual is participating in the in-person or phone redetermination process, and has not completed an application (BFA Form 800, Application for Assistance or BFA Form 800HR) or voice recorder signature and fails to:

o attend the scheduled interview appointment; or

o contact DHHS to reschedule an appointment.

· The individual fails or refuses to appear for a redetermination interview, if required (see PART 155 , REDETERMINATION ).
