002.02(C)(ii)(2)(a) DOCUMENTATION STANDARD.

The state review team (SRT) will review medical documentation dated no more than 12 months prior to the date for which a disability determination is requested. The individual whose disability status is being determined must have been examined by a physician, or appropriate provider, for the condition or conditions related to the disability determination within three months of the requested date.
