When an individual is determined eligible:

Enter the information in SPACES just like any other case with the following exceptions: Enter the address of the facility where the individual is residing for the individual’s address. Enter the Living Arrangement of ‘In Own Home’ with a date equal to the first day of the first month they began residing in one of the facilities.
