The assessment form is a standardized form that was designed for the State Agency MORE THAN A JOB NC program. The MTAJ-NC Provider completes the IEP with the participant. The plan identifies at least one career goal and steps to achieve the stated goal. After the assessment is conducted by the E&T provider, the completed MORE THAN A JOB NC assessment is uploaded into NC Works where it can be viewed by the State agency, providers, and the participant.
