Page 30 2024 Center-based Survey Instrument Thank you so much for participating in the Market Rate Survey! We are assisting the MS Dept. of Human Services by collecting information on child care tuition and the costs of providing child care services. The Department of Human Services is required to collect this information for federal funders. The survey will take about 30 minutes to complete, and you will receive a $25 gift card. You will also be entered into a drawing for $500 in gift cards. We will collect some identifying information from you so we can mark you off our list and also send you the gift card. However, your individual responses will be kept confidential and will only be available to our research staff. We will remove your identifying information from your answers before sharing with the MS Dept. of Human Services. Responses from all MS child care centers will be combined together for a report that will be made public on the MS Dept. of Human Services website this summer.
