The mailing address for SAH participants is Lot #XXXX, P.O. Box 17370, St. Paul MN 55117-0370. The “Lot” number is the only part of the address that will vary between SAH households. The “Lot” number is assigned to the household by SAH and provided to the Service Provider by the SAH applicant. Service Provider tool to help SAH participants complete application* When the application asks for personal information: Social Security Number Applicant should not provide a Social Security Number (for themselves or any household members). Name Applicant uses their real name on the application. Home Address Under “Home Address” applicant should list their Safe at Home Lot Number. Applicant should not fill in their actual home address. Mailing Address Applicant should use their assigned Safe at Home address on the application (Lot #, P.O. Box 17370, St. Paul MN 55117-0370) Applicant should not fill in their actual home address. When the application asks for income information: Proof of Income Applicant should remove or black out their workplace address, Social Security Number and actual home address from any copies of proof of income sent to meet this requirement. When the application asks for utility information: Copy of heating bill, electric bill or fuel receipt Applicant should remove or black out actual home address from any copies of their heating or electric bills sent to meet this requirement.
