All E&T providers have their E&T Case Manager act as the primary point of contact for participants. Case Managers are contractually required to check in with participants at minimum on a monthly basis in a real time communication. How do your offered case management services support the participant in the E&T program and provide activities and resources that help the participant achieve program goals? Case managers are encouraged to provide a robust and holistic approach to case management in working with this highly barriered population. How does the SNAP State agency ensure the case management services offered do not act as an impediment to successful participation in E&T?
