Required Documentation When submitting an IPV referral to OAH, the LAA must include a complete evidentiary packet containing all documents supporting the suspected IPV. A copy of this packet must also be provided to the accused applicant. Examples of required documents include: ● Intentional Program Violation Notice #1

● Referral for Investigation form

● Application(s) and all supporting documentation

● Prior year’s application and documentation (if applicable)

● Income verification results from systems such as The Work Number, E&E, SVES, or other approved databases

● Correspondence with the applicant and/or related parties

● Attestations or relevant statements from the applicant or stakeholders supporting the case Hearing Notification and Scheduling Upon receiving the IPV hearing request, the Office of Administrative Hearings (OAH) will issue a notice of hearing within three (3) business days to both the LAA and the applicant. ● The hearing will be scheduled no sooner than thirty (30) days from the date the OAH notice is sent. 5.5 Questionable Income and Fraud Investigations Questionable Income Applicant Investigations A.
