Volume 2 - Supplemental Nutrition Assistance Program (SNAP) MS 2905 – R. 6/30/26 – OMTL – 684 Verification of Citizenship
[Verification of citizenship is a technical eligibility requirement. An individual's statement is an acceptable form of verification, unless questionable.
If questionable, ask the household to provide verification of citizenship. Acceptable forms of verification include birth certificates, religious records, voter registration cards, certificates of citizenship or naturalization provided by the United States Citizenship and Immigration Services (USCIS), such as USCIS Form I-179 or USCIS Form I-197, or U.S. passports. If the above forms of verification cannot be obtained and the household provides a reasonable explanation as to why it is not available, accept a signed statement from someone who is a U.S. citizen that declares, under penalty of perjury, that the person in question is a U.S. citizen. For example: I, ______ _____, swear under penalty of perjury, I do know that
_ is a U.S. citizen.
For companion SNAP and Medicaid or Kentucky Transitional Assistance Program (KTAP) cases, the individual’s failure to present documentary evidence of citizenship for the Medicaid or KTAP case does not make the individual’s declaration of citizenship questionable for the SNAP case.]
