MS 0680 (2) 5. Keep mailed information secure: a. Check mail trays for recipient information regularly; and b. Do not leave recipient information in mail trays overnight. 6. Properly dispose of case record material and other recipient information as follows: a. Shred the material into 5/16 inch or smaller strips; or b. Place in a designated box: (1) Seal the designated boxes and store in a secure location, preferably one that can be locked; and (2) Complete the Certificate of Disposal form. B. Procedures for Safeguarding IRS Information – Computer Match: 1. When an IRS match is received on the computer, and independent verification is required, do not print any Worker Portal screens containing IRS data. These screens can be identified by the banner, which states, “This screen contains IRS data – do not print.” Do not specify the IRS data in case comments 2. If verification is available through system inquiry for the match, and no claim is established, contact the individual to verify the information is accurate, then file the information in the case record and comment accordingly. If verification cannot be obtained through system inquiry, or the individual cannot be reached, the case must pend for the required information. Allowable comments in the case concerning the resolution of the match would be “Computer Match dated ‘mm/dd/yyyy’ resolved”. Generic employment information should be entered for Request for Information (RFI) that are mailed as a result of a computer match. Example: While processing an income match for wages from ABC Employment Agency, staff are unable to reach the individual to confirm that the wages shown on Eligibility Advisor (EA) are representative of anticipated ongoing income. Enter generic employer information when entering into Worker Portal, such as, but not limited to, Job beginning xx/xx/xxxx. It is important that the employer’s name is not entered into the case until it is confirmed with the individual that the information is correct. The RFI must not state the name of the matched employer. 3. If verification returned as a result of the request indicates the need for a claim referral, follow normal procedures in establishing a claim. Note: Form PAFS-7, Notification of Appointment/Request for Verification, is obsolete effective May 1, 2011; however, the log used to track FTI-1 is kept in a locked file and maintained for five years after the last item on the log is destroyed, at which point it is destroyed per procedures found in General Safeguarding Procedures, item D.
