How to become a RCAP provider

Providers must meet one of the following criteria:

ISDH licensed residential care facility;

Christian Science Home;

ISDH licensed comprehensive nursing facility with residential beds;

County Home; or

DMHA approved with residential programs.

Providers must also have:

An active RCAP contract on file with the State of Indiana;

Disclosure for Housing with Services Establishment on file; if applicable.
