Case management is provided by E&T providers and Intermediaries and varies depending on provider. Describe how E&T case managers coordinate with other staff and services. Coordination can involve tracking E&T participation, sharing information that may be relevant to participation in E&T (e.g. information related to good cause or a work exemption), and referral to additional services. QUESTION RESPONSE FIELD How do E&T case managers coordinate with: SNAP eligibility staff During monthly provider meetings WFD staff and providers review participants information in ISETS. Any information required to be shared with the eligibility staff will be done by WFD staff via scheduled meetings, IES task or case notes in IES. How do E&T case managers coordinate with: State E&T staff Case managers will communicate with State E&T staff via the Illinois SNAP Employment and Training System (ISETS), during monthly staffings, or via email or phone. Information about the customers progress and participation in SNAP E&T along with potential barriers, may be discussed during these meetings How do E&T case managers coordinate with: Other E&T providers E&T providers are the State’s case managers, and they communicate with each other or State staff via information entered into ISETS, via email or phone. How do E&T case managers coordinate with: Community resources Case managers will communicate any needed information with community resources to provide the needed services to the customer. This information will be shared in several ways and will depend on the Provider and will be documented in ISETS. How does the State agency ensure E&T participants receive targeted case management services through an efficient administrative process, per 7 CFR 273.7(c)(6)(ii)? Case management will be delivered largely by our contracted provider agencies. Case management consists of one on one meetings to discuss an individual’s specific situation and the specific needs of the participant, challenges, and opportunities of that customer. Providers are required to keep case files on all participants and case notes from case management meetings should also appear in the files. These notes will be reviewed during the providers annual review to ensure that case management is being performed by every agency, for every SNAP E&T program participant. Provider contracts include the case management requirement and that at a minimum monthly case management notes are required in the case files. How do your offered case management services support the participant in the E&T program and provide activities and resources that help the participant achieve program goals? The way case management is provided varies by providers but is required at least monthly, some providers offer an individual approach while other offer a group approach or a combination of both. During case management the provider must discuss and offer participant reimbursements. How does the SNAP State agency ensure the case management services offered do not act as an impediment to successful participation in E&T? 1/28/26, 11:56 AM State Plan IL FY26 - Original Submission https://dam-fns.usda.gov/sites/default/files/microsite/snapet-ilfy26stateplan//index.html 28/268
