(HFS FHS) Contacts DentaQuest of Illinois, LLC to inform them of the appeal.

(DentaQuest) Works cooperatively with HFS to complete the appeals process, including preparing and sending a Decision Summary or Statement of Facts (Form 102) and representing the Department by telephone during the hearing.
