UNOFFICIAL 1700.1-52 a family of the size of the individual's family; (3) Has resources, as determined in accordance with SSI methodologies, that do not exceed twice the relevant maximum amount established, for SSI eligibility, for an individual or for an individual and the individual’s spouse; and (4) Is not otherwise eligible for Medicaid. “QEx” means the QUEST Expanded program that delivers medical and behavioral health services through health plans employing managed care concepts, to certain individuals in accordance with the State plan under Title XIX, or in accordance with a demonstration project under Title XI of the Social Security Act. “QExA” means the QUEST Expanded Access program that delivers medical and behavioral health services through health plans employing managed care concepts, to certain individuals who are aged, blind or disabled. “QMB” or “Qualified Medicare Beneficiary” means an individual who: (1) Is entitled to Medicare Part A, with or without payment of premiums, but is not entitled solely because the individual is eligible to enroll as a QDWI; (2) Has resources, as determined in accordance with SSI methodologies, that do not exceed twice the maximum amount established for SSI eligibility; and (3) Has income, as determined in accordance with SSI methodologies that does not exceed one hundred percent of the Federal poverty guidelines. “QMB only provider” means a provider of QMB services that is not certified to participate in the Medicaid program. “QMHP” or “qualified mental health professionals” means: (1) A psychiatrist licensed to practice medicine in the State of Hawaii in accordance with
