As part of the PE application, the DMA 285 - Third Party Liability Questionnaire is required for all applicants who have third-party liability coverage. Any coverage that pays for the cost of cancer treatment would make her ineligible. Limited scope coverage such as vision or dental coverage would not make her ineligible.
