Filing an Application 3.1 Submitting an Application To apply for benefits, a customer must submit an application with a minimum of name, address and signature using one of the following methods:  Online: Visit District Direct at https://districtdirect.dc.gov/ and submit the application electronically  Mobile: Download the District Direct Mobile App to your phone from the Apple or Google Play stores and use it to submit an application electronically  Mail an application form to the following address: DC Department of Human Services Case Record Management Unit P.O.
