Policy 2.008 1 | P a g e Title: Serving High-Risk Participants Category: Nutrition and Breastfeeding Education CFR Reference: 246.11 Policy Number: 2.008 Effective Date: 1/1/2016 Revised Date: 8/15/23
POLICY STATEMENT The State Agency must establish procedures to ensure adequate documentation and timely follow-up for participants designated high-risk. PROCEDURE 1. The State Agency establishes criteria for identifying participants with certain nutritional risk factors that require monitoring and follow-up by a CPA, see DC WIC policy 2.003 Nutrition and Breastfeeding Assessment. These risks are flagged in the MIS and are listed on DC WIC’s CPA Cheat Sheet.
2. At minimum, participants identified as high-risk must receive two individual nutrition education contacts by a CPA within three months of the initial identification of the high-risk condition. CPAs shall document the nutrition education contact in TGIF format, see DC WIC policy 2.005 Nutrition Education Overview.
3. The CPA may use their professional judgment to determine the frequency of monitoring and follow-up for each participant with a high-risk nutritional status.
4. In lieu of a CPA, a CPPA may complete a high-risk certification, but cannot provide education for risk codes that are deemed high-risk. The CPPA can counsel the participant on basic nutrition education and risk codes that are non-high risk. A high-risk participant may receive up to two months of food benefits between the certification and high-risk visit per the Local Agency discretion.
5. If a CPA determines through a subsequent nutrition assessment that a participant is no longer high-risk, the CPA may remove the high-risk designation in the MIS. The CPA must document the rational for removing the high-risk status.
