SUBJECT: Nutrition Care Plan

Federal Regulations: §246.11 (e)(5) WIC Nutrition Services Standards: Standard 14 POLICY A nutrition care plan shall be developed and implemented by a nutritionist for each high-risk participant and for each participant who wishes to have such a plan. Document the nutrition care plan in the Connecticut WIC Information System (CT-WIC). The WIC local agency should be consistent on where nutrition care plans are documented. It is preferred that nutritionists use the S.O.A.P. note format for the initial note for high-risk participants in the Nutrition Education screen. At a minimum, include a follow-up note describing the participant’s progress in achieving the agreed upon objectives. At the discretion of the nutritionist, a nutrition care plan does not need to be developed or implemented for an individual whose current condition does not warrant it. The reason(s) for such determinations shall be documented in CT-WIC Nutrition Education by the nutritionist.
