A certificate of U.S. citizenship (INS form N-560 or N-561); 4. A certificate of naturalization (INS form N-550 or N-570); 5. A certificate of birth abroad of a citizen of the United States (Department of State forms FS-545 or DS-1350); 6. Identification Cards for U.S. citizens (INS-I-179 or INS-I-197); or, 7.
