Language - No modification permitted to regulatory language; i.e., head of household, authorized representative (EDP and Manual).

Data Elements - No modification permitted except (1) serial numbers and photos may be added;

and, (2) data elements for signature of emergency authorized representative, number of persons in household, and household eligible for delivered meais may be deleted (EDP and Manual).
