ALABAMA INFANT FORMULA REFERENCE The Alabama WIC Program supports breastfeeding as the preferred method of infant feeding. For infants receiving formula follow the guidelines below. A complete list of Alabama allowed formulas can be found in the Alabama WIC Formulary with Qualifying Conditions and the Alabama Formula Max Quantity Report. Formulas are categorized according to USDA definitions. There are 3 categories: • Infant Formula (IF) – contract formula • Exempt Infant Formula (EIF) – formulas that require medical documentation • WIC Eligible Nutritionals (WEN) – formulas intended for children/woman to include modular formulas Individual formulas are listed under subcategory within one of the above categories. RFO = reconstituted fluid ounces • 1 - 13 oz. can concentrate yields 26 RFO • 1 - 12.4 to 16 oz. can powder yields 85 to 115 RFO (Powdered formulas reconstitute to different fluid ounces per can) • 1 - 32 oz. can ready to feed (RTF) yields 32 RFO • 1 – 8 oz. can ready to feed (RTF) yields 8 RFO Formula is prescribed in Crossroads in ounces. WIC regulations state that full nutrition benefit (FNB) should be issued to each participant per category. Fully formula fed infants receiving 8 oz. ready to feed formula may receive more than the FNB for other types of formula but the total amount is within the Maximum Monthly Allowance (MMA) for ready to feed formula. Contract formulas are: • Enfamil Infant – 12.5 oz. powder and 13 oz. concentrate • Enfamil Gentlease – 12.4 oz. powder • Enfamil Prosobee – 12.9 oz. powder and 13 oz. concentrate • Enfamil AR – 12.9 oz. powder • Enfamil Reguline – 12.4 oz. powder Formula amounts that may be issued for contract formula based on age and category are as follows: Maximum amounts for Infant Partially Breastfed1 Maximum amounts for Infant Fully Formula Fed 0 through 3 months – 4 cans powder 2 0 through 3 months – approximately 26 fl oz./day (364 RFO) 14 cans 13 oz. concentrate (806 RFO) 9 cans powder 2 12 cans 32 oz. RTF
31 cans 13 oz. concentrate
26 cans 32 oz. RTF 4 through 5 months – 5 cans powder 2 4 through 5 months – approximately 29 fl oz./day (442 RFO) 17 cans 13 oz. concentrate (884 RFO) 10 cans powder 2 14 cans 32 oz. RTF 34 cans 13 oz. concentrate
28 cans 32oz. RTF 6 through 11 months – 4 cans powder 2 6 through 11 months – (approximately 20 fl oz./day) (312 RFO) 12 cans 13 oz. concentrate (624 RFO) 7 cans powder 2 10 cans 32 oz. RTF 24 cans 13 oz. concentrate
20 cans 32 oz. RTF NOTE: Medically fragile infants 6 to 11 months of age may receive the same maximum monthly allowance of contract infant formula or exempt infant formula as infants 4 through 5 months of age on the same feeding option, i.e., partially (mostly).
1If the infant is partially breastfed, the least amount of formula should be issued to encourage continued breastfeeding. 2The number of cans of powder formula that may be issued varies depending on the reconstituted fluid ounces (RFO) specific for the formula. RFO for each formula issued per age and category is listed on the AL Formula Max Quantity Report. See Chapter 9, Reports for instructions on printing this report monthly.
