Refer to Alabama WIC Procedure Manual, Chapter 9 Reports for instructions (instructions also provided below). o Provide/Review High Risk Nutrition Care Plan is a monthly WIC Coordinator responsibility. o The RD must run the AL High Risk Participant Report for the previous month. For example, when running the report in April run from 3/1 to 3/31. ▪ Review the High Risk participant record ensuring that High Risk Care Plan Guidance has been followed. ▪ The WIC Coordinator (RD) should document “HR Follow Up Review” in the Nutrition Assessment field of the Care Plan Summary noting whether the Care Plan is appropriate or additional follow up and counseling is needed. ▪ Additional follow up by RD may be in person or remote based on the individual participant needs. Document follow up visits and/or attempts to contact in the Nutrition Assessment field of the Care Plan Summary. ▪ Place check mark by name when complete and file in High Risk folder.
